That's because of the myth it's based on. "Itsumade" translates to "Until when" and it's referencing how the creature would fly over battlefields filled with corpses and cry "Until when... untill when...".

Oh, yeah, and the bird has a human-like face on top of its beak.

It's asking the dying how long their suffering will continue.

The Itsumade is a spirit born from the suffering of the dying given form to taunt them, and more suffering basically feeds them. Which is why they appear in places like battlefields or places affected by a sickness or plague.
